    • To remain compliant to Borough's licensing regulations we are required to keep treatment records for inspection by the London Borough of Hillingdon for a period of 2 years. In these consultation forms we collect the following information.

    • ​Your Name, age, postal address, email address (if available) and contact phone number.

    • Type of treatment given and details of any previous treatments.

    • Name of the therapist who has provided the treatment,

    • Details of any medical conditions, allergies or other contradictions.

    • Your informed consent and signature.

  • ​At My Beauty Basics, we maintain client records and treatments history to provide and improve our services to the customers.

  • We also process data to manage ongoing business operations.
